Patient Information

Booking Appointment

In general, consultations are by appointment and are made by telephoning 8346 2626. Please indicate your preferred time and the doctor you wish to see. Telephone consultations are also Please call the clinic first if you have cough, cold or flu like symptoms.


Emergencies and urgent problems are given priority. If your problem is urgent, please advise the reception staff to ensure that appropriate arrangements are made. Walk in patients for emergency only.


If you are unable to keep your booked appointment, please telephone our receptionists. A non-attendance fee will be charged for appointments not cancelled at least 24 hours in advance. 


Please let the receptionist know when you book, if you require a surgical procedure, life insurance, sports medical examination etc. Longer appointments are available on request, subject to requirements and approval by the doctor.

Tele-Health Appointments

Tele-Health Appointments are available to support you with the management of your health if you have upper respiratory symptoms or think you may have Flu or COVID -19 the reception staff will help you to make an appointment and advise you regarding our Tele-Health appointment system. We bulk bill Tele-Health consultations.

After Hours Services

The Hello Home Doctor will support you with any afterhours care that you may require, they are accredited Medical Deputising Call the National Home Doctor Service on 134 100 for after hour services.

Telephone /Electronic Communication

It is our practice policy that we do not answer clinical questions by email. To comply with RACGP accreditation standards our clinic will not accept any correspondence electronically .i.e. via email, SMS, social media) from patients, relatives and friends of patients, medical providers, insurance companies etc requesting or requiring medical advice or information. For medical advice please call reception to make an appointment with your GP.

Languages spoken: Chinese, Mandarin, Cantonese and Vietnamese.

Behaviour

We understand that you may be unwell, stressed or anxious. We have a zero-tolerance policy towards violence and will not tolerate verbal or physical threats made towards members of the staff or General Practitioners.

Free parking is available onsite, with additional paid parking nearby.

Disabled Access

 Access for people with disabilities is available.​

Sickness Certificates, Repeat Prescriptions and Referrals

Sickness Certificates, repeat prescriptions and referrals require an appointment. Please be aware that the doctors are unable to back date medical certificates and referrals.

Privacy & Confidentiality

Our practice adopts the Australian Medical Association (AMA) code of ethics. All patient consultations and medical records are kept strictly confidential and patient documents and private information do not leave the clinic without patient permission. It is our practice policy to maintain security of personal health information at all times and that information is only available to authorised staff members. If you would like a copy of our Privacy and confidentiality Practice Policy, please ask the reception staff who will be able to provide you with a copy. 


All medical records are stored electronically and are protected by sophisticated password systems and access to our computer system is not available outside of the practice. As part of our Privacy Policy written consent is required to access any medical information contained in patient health records. If you have any concerns, you can contact the National Privacy Hotline on 1300 363 992. 

Smoking

 A non-smoking policy exists throughout our clinic.

Your Rights and Feedback

The Doctors and support team of the Croydon Medical Centre always aim to provide you with the best possible health care service. If you have a suggestion about our service, or if you are unhappy with any aspect of it, we would encourage you to talk with your doctor, or ask to speak to the Practice Principal Dr Stephanie Vuong. We take your feedback seriously, however, addressed to Health and if we are unable to resolve your concerns, complaints can be addressed to Health and Community Services Complaints Commissioner SA Phone: 08 8226 8666 www.hcscc.sa.gov.au PO Box 199, Rundle Mall SA 5000). Staff can also provide you with a form to complete regarding any issues and complaints that you may have which will be actioned within 7 business days.
